EMPLOYEE BENEFIT SUMMARY

    The following are some benefits available to all regular full-time employees:

    PAID TIME OFF

Holidays - Sonus provides the following holidays: President's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and the day after, and Christmas Day through New Year's Day.

Vacation - Vacation accrual begins with the first day of employment. Accrual rate for full-time employees is 3 weeks (15 days) per year, and for employees with more than 5 years of service, the accrual rate increases to 4 weeks (20 days) per year. The accrual rate is prorated for part-time employees.

Sick Time/Leave of Absence - Full-time regular employees may be eligible for salary continuation benefits for short-term absences due to illness or injury.


HEALTH INSURANCE BENEFITS

Medical and Dental Insurance - All regular employees who work a minimum of 20 hours per week are eligible to participate in our health care insurance plans. Sonus pays 100% of the medical, dental and vision premiums for our employees, and 90% for dependent coverage. Employees may choose from 3 medical plans.

STOCK OPTIONS

All full-time employees are eligible to share in Sonus Pharmaceutical's success as participants in our Stock Option Program, at the discretion of the Board of Directors.

Stock options generally vest over 4 years, with 25% vesting after 1 year and the remaining vesting in equal installments over the next 36 months.


401K PLAN

401(K) Plan - All regular employees are eligible to participate in Sonus Pharmaceuticals' 401(K) Plan. The company offers a corporate match up to 2%, paid in SNUS stock.


LIFE AND DISABILITY INSURANCE

All regular employees who work 24 hours or more per week are eligible for $50,000 life insurance and Accidental Death & Dismemberment coverage through Unum. Short Term Disability Insurance and Long Term Disability Insurance are offered through Standard Insurance, which pays 60% of weekly earnings in the event of a disabling illness or injury.
 

EMPLOYEE STOCK PURCHASE PLAN

All regular employees who work 30 hours or more per week are eligible to participate in the company Stock Purchase Plan. Enrollments occur quarterly.


FLEXIBLE SPENDING PLAN ACCOUNTS

Sonus pays 100% of the fees to sponsor this plan which allows employees to set aside pre-tax dollars to be used later as personal reimbursement funds for eligible medical-related and dependent care expense.


EMPLOYEE ASSISTANCE PROGRAM (EAP)

EDUCATIONAL ASSISTANCE

Employees may request tuition reimbursement for the cost of educational courses which improve their job-related skills. Approval must be granted prior to enrollment.


OTHER (MISCELLANEOUS)

Professional Dues -Sonus may pay for professional memberships for job related organizations.

Health Club Dues -Sonus reimburses $100 per year per employee for health club membership dues.
